Under-Eye Concerns
Treats hollow tear troughs, dark circles caused by shadowing, and the “tired look” that concealer can’t fix. Dark circles caused by pigment rather than hollowness respond better to pigment treatments, and puffiness or festoons need a different plan entirely — your injector will tell you honestly which category you’re in.
Ideal Candidate
Best for patients with genuine volume loss under the eyes — a visible groove between lid and cheek — and reasonably thick, healthy under-eye skin. Not ideal for significant puffiness, very thin or crepey skin (consider PRF instead), or malar edema. Not performed during pregnancy or breastfeeding.
Filler vs. PRF vs. EZ Gel
Under-eye assessment separates hollowing from pigment, puffiness and skin changes. The options are HA filler ($600), PRF ($700) and EZ Gel ($900), but they are different procedures with different evidence and risks—not interchangeable solutions for every dark circle.

Under-Eye Concerns We Treat
The under-eye is the most nuanced injection area on the face. Getting the diagnosis right matters more than the syringe.
Hollow Tear Troughs
The classic candidate: a visible groove from the inner corner of the eye toward the cheek. Deep-plane HA filler restores the support, smoothing the transition in one appointment. Results last 9–18 months in this low-movement zone.
Dark Circles (Shadow Type)
If your dark circles fade when you gently pull the skin taut or look up toward light, they’re shadow-driven — and filler fixes the cause. If they persist in every light and angle, they’re pigment-driven; your provider may point you to brightening treatments instead, or combine approaches.
The “Always Tired” Look
When colleagues keep asking if you slept, the culprit is usually the lid-cheek junction. Restoring it is one of the highest-impact, most natural-reading treatments in aesthetics — nobody can say what changed, only that you look rested.
Thin, Crepey Under-Eye Skin
Crepey skin, pigmentation and puffiness are different from a volume hollow. PRF or other options may be discussed after assessment, but an injectable is not automatically the best answer for every concern.
Genetic Hollows in Younger Patients
Tear troughs aren’t only age-related — some anatomies show them from the twenties. Young patients with good skin are often the cleanest correction cases, and results can stretch toward the 18-month end of the range.
Previous Under-Eye Filler Problems
Puffiness, a blue tint or a lump after prior filler needs assessment. Hyaluronidase may help reduce HA filler, but complete correction is not guaranteed and refill timing depends on the cause and healing.
Under-Eye Filler vs. PRF
Both treat the under-eye — one adds volume immediately, one rebuilds quality gradually. The right choice depends on your anatomy and timeline.
What to Expect
A precise, unhurried 30–45 minutes — most of it assessment and mapping, because under-eyes reward caution.
Assessment & Honest Triage
Your injector determines whether your dark circles are hollow-driven, pigment-driven, or puffiness-driven — and tells you plainly if filler isn’t your best option. Photos in multiple lighting angles set the baseline.
Numbing
Topical numbing is applied to the entry point area. The cannula technique needs only one small entry per side, so numbing is quick and effective.
Cannula Placement
The injector chooses the product, tool and placement according to your anatomy and treatment plan. Small amounts are assessed carefully, but a cannula does not eliminate vascular or other complications.
Review & Aftercare
The hollow is visibly softened on the spot. Expect possible mild swelling and, less commonly with cannula, light bruising at the entry point. Cold compress guidance and aftercare are reviewed.
Follow-Up & Refinement
Swelling and settling vary, and persistent puffiness or irregularities need assessment. A follow-up reviews healing before any additional product is considered; confirm the clinic’s timing and cost terms.
Post-Treatment Tips
Follow product-specific guidance on pressure, exercise, makeup and other procedures. Do not stop prescribed blood thinners without advice from the prescribing clinician. Swelling, bruising, tenderness and lumps can occur. Rare vascular complications can cause skin damage, vision loss or stroke. Seek immediate medical attention for unusual severe pain, white/gray/blue skin, vision changes or stroke symptoms after filler; do not wait for a routine follow-up. How Many Sessions & How Often
The amount should be individualized to the hollow and existing product. Reassessment after healing helps determine whether any further treatment is appropriate.
The under-eye barely moves, so filler outlasts most other areas. Many patients refresh annually; some stretch to 18 months.
If skin quality is part of the picture, your injector may sequence PRF or EZ Gel sessions between filler refreshes for compounding improvement.
Under-eye work pairs naturally with Botox for crow’s feet and cheek filler — supporting the mid-face often reduces how much product the tear trough itself needs.
How Under-Eye Filler Works
The tear trough is a groove that forms (or deepens) as the fat pads and bone support under the eye recede with age. Light falls into the hollow and casts a shadow — which is why many “dark circles” are really a volume problem, not a pigment problem.
Under-eye filler adds volume to selected hollows between the eyelid and cheek. The tool and placement are chosen for the individual, but serious vascular complications remain possible with needles or cannulas.
The half-syringe option is $600. The appropriate amount depends on anatomy and existing filler; it is not a standard dose for every tear trough. Confirm the product, price and follow-up terms at consultation.
