The lip flip has no formal downtime. You walk out of the appointment and resume your day. But “no downtime” doesn’t mean “no instructions.” There’s a short list of restrictions in the first 24–48 hours that exist for a specific clinical reason — to keep the neurotoxin in the intended treatment area and to minimize the already-small risk of side effects.

This post covers every aftercare instruction for the lip flip, explains the clinical reason behind each one so you understand why it matters rather than just following a list, and provides exact timelines for when each restriction lifts. If you’ve already had your lip flip and you’re reading this with a specific question — “Can I work out tonight?” “Can I drink wine?” “When can I kiss someone?” — scroll to the relevant section. The answer is here.

The First 4 Hours: The Critical Window

The first 4 hours after a lip flip are the most important from an aftercare perspective. This is when the neurotoxin is binding to the nerve receptors at the neuromuscular junction but hasn’t yet been fully absorbed. During this window, physical manipulation of the treatment area can theoretically cause the product to diffuse beyond the intended injection site — affecting adjacent muscles and producing unintended effects.

The restrictions during this period are simple and non-negotiable.

Don’t touch, press, rub, or massage the treatment area. Your hands should not contact the skin above or around your upper lip for 4 hours. This means no scratching an itch (use the back of your hand or a tissue if needed), no applying pressure when wiping your mouth, no absent-minded lip touching. The concern is that external pressure on the injection sites can displace the neurotoxin before it has fully bound to the target receptors.

Don’t lie down. Stay upright — sitting or standing — for at least 2 hours after treatment. The gravity-related concern is that lying flat can encourage the neurotoxin to migrate downward from the injection sites. With forehead Botox, this instruction exists to prevent drooping eyelids. With a lip flip, it exists to prevent the product from affecting muscles below the treatment area, which could cause asymmetry or unintended lip weakness.

Two hours is the minimum. Four hours is more conservative and preferred if your schedule allows it. You don’t need to stand perfectly still — sitting at a desk, driving, walking around, running errands are all fine. Just don’t recline or nap.

Don’t apply makeup or products to the lip area. Applying lipstick, lip gloss, lip balm, or any topical product to the injection area within the first 4 hours introduces pressure (from the application itself) and foreign substances to tissue that has open micro-punctures from the injections. Wait 4 hours before applying anything directly to the lips or the skin immediately above them.

Eating and Drinking

This is the most common post-treatment question, and the answer is more nuanced than a simple yes or no.

Can I eat after a lip flip? Yes, but with awareness. You can eat immediately after your appointment — there’s no fasting requirement. However, be mindful of two things in the first few hours. First, avoid pressing your lips together forcefully when biting or chewing. Aggressive lip compression during eating creates pressure on the treatment area that mimics the “don’t touch” restriction. Second, avoid extremely hot foods and beverages for 4–6 hours. Heat increases blood flow to the area, which can theoretically accelerate diffusion of the neurotoxin and increase minor swelling at the injection sites.

Soft foods that don’t require aggressive biting — soup, yogurt, pasta, soft bread — are the easiest choices for your first meal after treatment. By the next meal, eat whatever you want normally.

Can I drink through a straw? You can try, but you may find it awkward. This isn’t an aftercare restriction — it’s a functional consequence of the treatment itself. The lip flip relaxes the muscle that creates suction, so straw use may feel less effective starting a few days after treatment as the Botox takes effect. In the first 4 hours specifically, the concern with straws is the pursing motion, which contracts the orbicularis oris and creates pressure in the treatment area. Use a cup or glass for the first 4 hours, then decide based on how straws feel once the muscle relaxation sets in.

For a complete breakdown of functional changes including straw use, speech, and eating, see our post on lip flip side effects.

Can I drink alcohol? Avoid alcohol for 24 hours after treatment. Alcohol is a vasodilator — it widens blood vessels and increases blood flow. This can worsen bruising at the injection sites and may contribute to minor swelling. One glass of wine at dinner won’t cause a complication, but it increases the likelihood of visible bruising the next morning. If you can avoid it for 24 hours, do so. If you have a social obligation the evening of your appointment, limit consumption and understand you may bruise slightly more.

Can I drink coffee or hot beverages? Wait 4–6 hours. The heat issue applies to any hot liquid — coffee, tea, hot chocolate. The temperature increases blood flow to the perioral area. Room temperature or iced coffee is fine immediately. Hot coffee can wait until the afternoon if your appointment was in the morning.

Exercise

Can I work out after a lip flip? Avoid strenuous exercise for 24 hours. This is one of the most commonly violated aftercare instructions and one of the most important to follow.

The reasons are physiological. Intense exercise increases heart rate and blood pressure, which increases blood flow throughout the body — including to the treatment area. Elevated blood flow can cause the neurotoxin to diffuse beyond the injection site more readily, potentially affecting adjacent muscles and increasing bruising risk. Additionally, exercises that involve significant facial strain, bending forward (inversions in yoga, deadlifts, burpees), or positions where your head is below your heart can increase pressure in the facial vasculature.

After 24 hours, all exercise is fine — including high-intensity workouts, weight training, running, and hot yoga. The neurotoxin has bound to the receptors by this point and won’t be displaced by physical activity.

What counts as “strenuous”? Anything that elevates your heart rate significantly — running, cycling, HIIT, weight lifting, vigorous swimming, hot yoga, competitive sports. A casual walk, gentle stretching, or light housework is fine on treatment day. If you’re questioning whether it counts, wait 24 hours.

If you have a workout routine you can’t skip: Schedule your lip flip appointment after your workout, not before. Work out in the morning, get treated in the afternoon, and the 24-hour restriction carries you to the next afternoon — you only miss one session at most.

Kissing and Physical Contact

This is the question patients want to ask but often don’t. The answer is straightforward.

Avoid kissing for 4 hours after treatment. Kissing involves direct pressure on the lip tissue — exactly what the first 4 hours of aftercare is designed to prevent. A quick, light peck is unlikely to cause a problem, but any sustained or firm lip contact creates the kind of pressure that could theoretically displace the product before it’s fully bound.

After 4 hours, kiss normally. The neurotoxin is sufficiently absorbed by this point that external pressure won’t affect its placement. Your partner won’t feel anything different about your lips — there’s no product in the lip tissue itself (unlike filler, which adds a physical gel). The Botox is in the muscle above the lip, not in the lip body.

Dental Appointments

Avoid dental work for 2 weeks after a lip flip. Dental procedures involve significant manipulation of the perioral area — opening the mouth wide, retracting the lips, direct contact with instruments near the injection sites. In the first 2 weeks, while the neurotoxin is still actively taking effect and stabilizing, this level of manipulation can potentially disrupt the result or cause uneven diffusion.

If you have an upcoming dental cleaning or procedure, schedule it either before your lip flip or at least 2 weeks after. If an emergency dental situation arises, the treatment won’t complicate dental care in any clinically meaningful way — but your lip flip result may be slightly affected.

Skincare and Facial Treatments

Resume normal skincare after 4 hours. Your regular cleansing and moisturizing routine can resume after the initial 4-hour restriction lifts. Avoid aggressive exfoliants (scrubs, acids, retinol) directly on the upper lip area for 24 hours, as the micro-puncture sites are still healing during that window. Everywhere else on the face is fine immediately.

Wait 2 weeks for facial treatments. HydraFacial, chemical peels, microneedling, and other facial treatments that involve direct contact with or product application to the perioral area should wait at least 2 weeks. These treatments create pressure, heat, or chemical activity on the skin surface that could theoretically interact with the neurotoxin placement. Facial treatments that avoid the mouth area entirely (an eye-focused peel, for example) can be scheduled sooner, but discuss timing with your provider.

Facial massage: Avoid for 48 hours. No facial massages, gua sha, jade rolling, or lymphatic drainage techniques on or near the lip area for at least 2 days. The mechanical pressure can displace the neurotoxin.

Sleep

Sleep normally, but not within 2 hours of treatment. The upright restriction applies for the first 2 hours. After that, you can lie down and sleep in any position. There’s no need to sleep elevated (as there is with lip filler, where swelling is a more significant concern). The lip flip involves minimal to no swelling, so sleep position after the initial 2-hour window has negligible impact on the result.

If your appointment is in the late afternoon or evening, just make sure 2 hours pass before you go to bed. Morning and midday appointments eliminate this concern entirely.

Sun and Heat Exposure

Avoid direct heat for 24 hours. Saunas, steam rooms, hot tubs, hot showers (prolonged), and sunbathing should wait 24 hours. Heat causes vasodilation, which increases blood flow to the treatment area and may contribute to product diffusion and minor swelling. A normal-temperature shower on treatment day is fine — just avoid lingering in very hot water.

After 24 hours, no heat restrictions apply. Normal sun exposure, saunas, and hot tubs are all fine. The neurotoxin is fully bound and unaffected by temperature at that point.

Other Common Questions

Can I fly after a lip flip? Yes. There’s no restriction on air travel. Cabin pressure doesn’t affect neurotoxin placement. If you’re flying the same day as treatment, just follow the standard aftercare (upright for 2 hours, no touching, no alcohol on the flight).

Can I smoke or vape? You can, but the pursing motion involved in smoking and vaping engages the orbicularis oris — the muscle the lip flip is relaxing. In the first 4 hours, avoid it to prevent pressure on the treatment area. After that, the functional issue takes over: you may find the pursing motion feels slightly different or less controlled as the muscle relaxation sets in. This isn’t a complication — it’s the treatment working on the exact muscle you’re using.

Can I take pain medication? Ask your clinician or pharmacist which pain reliever is appropriate for you. Some medicines can increase bruising, but a general aftercare timetable is not a reason to interrupt medically necessary treatment. Do not stop, restart, or change prescribed aspirin, anticoagulants, or other medicines for a cosmetic appointment unless the clinician who prescribes them tells you to. Read the NHS guidance on anticoagulant medicines.

Can I get other Botox treatments the same day? Yes. Many patients at ELLEMES receive the lip flip alongside Botox for forehead lines, crow’s feet, or a brow lift in a single appointment. The aftercare instructions overlap — follow the most conservative version of each (so the 4-hour no-touch restriction applies to all treated areas). Your injector coordinates the treatment plan so that all areas are addressed in a single visit when possible.

Can I get lip filler the same day? Yes, though the aftercare changes when filler is added. Lip filler has a more extensive recovery profile — more swelling, bruising, tenderness, and a longer list of restrictions. If you receive both treatments at the same appointment, follow the lip filler aftercare instructions (which are more conservative and encompass everything on the lip flip aftercare list). For the full filler recovery timeline, see our post on lip filler swelling stages.

The Complete Quick-Reference Timeline

For easy reference, here’s when each restriction lifts.

Immediately fine: Eating (soft foods, room temperature). Drinking (water, cool beverages). Driving. Working (desk work, non-physical tasks). Talking. Walking.

After 2 hours: Lying down. Sleeping.

After 4 hours: Touching the lip area. Applying lipstick, lip balm, or makeup to the lips. Kissing. Smoking or vaping. Hot coffee or tea.

After 24 hours: Strenuous exercise. Alcohol. Ibuprofen or aspirin. Hot showers, saunas, hot tubs. Aggressive skincare on the lip area.

After 48 hours: Facial massage. Gua sha or jade rolling near the mouth.

After 2 weeks: Dental work. Facial treatments that contact the perioral area (HydraFacial, chemical peels, microneedling).

What Happens If You Break a Rule

Aftercare instructions are risk-reduction measures, not guarantees. Breaking one doesn’t mean your lip flip will fail or that you’ll experience a complication. It means the probability of an unintended effect — product diffusion, asymmetry, increased bruising — goes up slightly.

If you accidentally touched your lip area within 4 hours, or exercised on treatment day, or had a glass of wine that evening — it’s done, and the most likely outcome is that everything is fine. Don’t panic, don’t try to “fix” it, and don’t cancel your next appointment out of guilt. Monitor for any unusual symptoms (worsening asymmetry, unexpected muscle weakness beyond the lip area) and contact your provider at ELLEMES if anything seems off. But in the vast majority of aftercare-violation scenarios, the result is completely unaffected.

The instructions exist because following them gives you the best possible chance of an optimal result. They don’t exist because violating them causes guaranteed harm.
